Transitional Post-Acute Care Program
Through our swing bed program, known as the Transitional Post-Acute Care (T-PAC) Program, patients can safely and comfortably move from an acute hospital stay to the next level of care with continued skilled nursing and rehabilitation services.
Every patient is a valued member of their own care team. This team includes physicians, therapists, registered nurses, nursing assistants, nutrition services, and pharmacists—all working together to support each patient’s unique recovery and goals.
Patients receiving out-of-town care can return to Delta Health for skilled nursing and swing bed services. Our goal is to help community members maintain quality of life while staying close to home, family, and friends.

IS THE CARE DIFFERENT?
In an acute hospital setting, patients receive a higher level of care focused on meeting critical needs and treating emergent conditions.
In the Transitional Post-Acute Care (T-PAC) program, the focus shifts toward preparing patients for a safe return home. Patients are encouraged to participate in activities of daily living—such as dressing, bathing, and grooming—in a supportive, home-like environment. They also take part in therapeutic exercises and recovery activities designed to build strength, independence, and confidence.
While the level of intensity differs from hospital-based care, the quality remains the same. The dedicated staff at Delta Health’s T-PAC program partner with each patient to create a care experience that supports recovery and promotes a successful return to home and community life.
WHAT TO BRING:
The T-PAC Program is designed to help patients transition safely from hospital care to home (or to a lower level of care). Each day, patients actively participate in their recovery through routines that build confidence and independence at home. With guidance from their care team, patients practice daily activities such as dressing, grooming, and other activities of daily living (ADLs).
To support comfort and recovery, we encourage patients and families to bring:
- Comfortable walking
- or tennis shoes
- Casual, comfortable clothing
- Electronic devices or phones
- Books, e-books, or notepads
- Small personal items from home
Paying for the T-PAC Program
The Transitions Post-Acute Care (T-PAC) Program, also known as “Swing Bed Program,” is covered by Medicare.
To qualify for Medicare A coverage, patients must have been admitted to the hospital under “inpatient” status for at least three consecutive days within the past 30 days.
Patients transition out of the program once the rehabilitation team determines that all reasonable goals for improvement have been met. T-PAC is covered by Medicare and select Medicare Advantage plans.
