COVID-19 Vaccine Booster Clinic (Moderna & Pfizer)

Delta Health is hosting a Pfizer and Moderna COVID-19 Booster Clinic for those ages 16 and older on December 17th from 8AM-12PM.
No walk-ins. Space is limited and you must have an appointment. Please bring your vaccine card and your insurance card. Masks are required.
Where: 100 Stafford Lane in Delta, CO inside the Grand Mesa Medical Building (Please use the Urology Clinic Entrance)
When: December 17th, 2021
