COVID-19 Vaccine Clinic (Johnson & Johnson)

The Johnson & Johnson COVID-19 vaccine is authorized to be used in those 18 years of age or older.

Please review the available slots below and click on the button to sign up. As a reminder, this is the Johnson & Johnson vaccine. Print off the attached Vaccine Screening Form, and bring the demographic and screening questions completed to the clinic. The consent portion of the form will need to be completed at the vaccine clinic. Printing this form is not required but will speed up the process.

There is no cost to receive the vaccine and no requirement to have insurance. We ask for insurance cards only because some insurance companies reimburse us $40 for administering the vaccine. If you have questions let us know!
