Update on Delta Health Clinics

Delta Health Clinics strive to make preventative health care our priority and continue to explore avenues to make it a positive experience for our patients.

During 2016 the clinics have undergone many changes. All of the Delta Health owned clinics share the same Athena Electronic Medical Record (EMR) which has improved patient care between the clinics. Athena has improved communication between the providers, the clinic staff, and the patient.

The patient portal has been a quick and efficient way to communicate with the patients. Through the clinic portal patients now have the ability to pre-check-in for appointments, text/email/phone in to set up appointments or for prescription renewals. As we continue to navigate in the EMR world we continually explore Athena programs that will improve patient care and access.

The Clinic Managers meet on a regular basis to improve their clinics and to streamline the patients experience in the clinic. In addition when one clinic is short staffed with clinical professionals or providers, staff from other clinics assists in covering the shortages. One of our goals is to standardize the same processes and policies in each clinic. The patient application on line is now only 3 pages compared to the previous 8 page application. We were able to streamline this form because we are doing In-Takes for a new patient. The applicant will be notified they have been accepted as a patient and they will be called to set up an In-Take appointment. There will not be a charge for this appointment; this appointment will be with clinical staff to gather the needed data. We have found this to be great introduction to the clinic for the patient. With the information loaded in the chart it has proven to be a smoother transition between the patient and provider.

We continually research and interview providers to bring to our community so we can provide health care to all of Delta County residents. We have great providers whether they are Doctors or Nurse Practitioners.

The ground breaking for the clinic in Hotchkiss has already taken place and we are excited about serving the North Fork Valley with a great location and a larger clinic, which is anticipated to be opened by mid-year 2017. We are also exploring potential locations in Paonia to place a Walk-In/Urgent Care Clinic to serve additional North Fork patients.

In 2016, behavioral health has been integrated into Delta Health Family Medicine Clinic in Delta and will be gradually integrated into the other primary care provider clinics in 2017.

Also during the coming year, in 2017, the Delta hospital clinics are partnering with Delta County School District to expand opportunities for student shadowing that may be interested in health-related education and careers. A few of the opportunities are in areas such as coding, information systems, imaging, OB-GYN and clinical staffing such as CMA, CNA, LPN or RN.

Delta Health has partnered with Montrose Memorial Hospital Gunnison Valley Hospital, and Aspen Valley Hospital to organize the San Juan Accountable Care Organization (ACO). The San Juan ACO is a program that utilizes a team-based approach focused on continuous partnership between the patient, family, caregiver, physician, hospital and community resources to provide longitudinal, coordinated care for complex patients by embedding a care coordinator in the community and/or practice being served. Our hospital has employed three experienced, professional Registered Nurses who are patient care coordinators that are working in chronic care management of Medicare patients..

Delta Medical, located at 70 Stafford Road (across from the Oncology Building parking lot) recently has been converted into offices and exam space for rotating specialist providers to see their patients. The following specialist providers are seeing patients at the Delta Medical Building at 70 Stafford Road on a rotating basis:

Grand Valley Foot and Ankle – Monday (every other)

Dr. Burnbaum – Neurology – Wednesday (every other)

San Juan Urology – Thursday (every other)

Western Slope Cardiologists – Tuesday, Wednesday, and/or Thursday (this also varies from week to week)

Meditronics – PACE Technician Tuesday, Wednesday, and/or Thursday (this also varies from week to week)

Boston Scientific – PACE Technician Tuesday, Wednesday, and/or Thursday (this also varies from week to week)

Dr. Maruca – Endocrinology – Friday (every other)

Dr. Good – Psychiatrist is still practicing out of Internal Medicine every Tuesday.

Please note if patients need to cancel or schedule appointments they need to contact that specialty provider’s individual clinic directly.
